Virtual Doctor Visits
Getting Started
To get started you will need to call our office to schedule an appointment. Please be sure to let the receptionist know you are scheduling a virtual appointment. You will be pre-screened by our staff to ensure that you qualify for a virtual appointment. On rare occasions, the provider may decide a phone visit is adequate to treat you. After your appointment is made, you will need to read and sign our Virtual Medicine Acknowledgement form, located above. Email the completed form to [email protected]. Once the acknowledgment form is signed, we will verify your benefits for the visit. We will call you to let you know the cost. You can either pay over the phone at the time or click the “pay online” button above at your convenience prior to your appointment. After these few steps are complete and on the day of your appointment, you will get an email from us with a link for prompts to connect with the provider. Fifteen minutes prior to your appointment, please call our office to “check-in.” This time allows both you and the provider to get ready for your virtual visit.
If you already have an existing appointment and would like to convert your regular office visit appointment to a virtual visit, please call our office first to see if you qualify.
